The Florida High School Activities Association requires that students in grades 6-12 participating in a JH, JV and Varsity sport, have a completed FHSAA physical form by his/her physician. Parents are to turn this form in to the athletic office along with a signed Consent Release form prior to participating in any practices and games.

The attached FHSAA sports physical form is the only form accepted for student athletes to be able to participate in athletics.

The following Consent Release Form is to be completed and signed by the parent, and is to be attached with the sports Physical Form as required by the state. No student athlete is permitted to participate in a school represented practice and a school represented game without both valid forms.
